Health & Safety Class Registration

In order to register for a Health & Safety course, you must

complete steps 1 & 2.

Step 1 - Complete the registration form below.  Upon submitting your registration, you will be linked to a confirmation page and step 2.

Payment is due at the time of registration, prior to the class.

Step 2 - Two payment options are available - please have your credit card information available.

1) Call the chapter and pay over the phone OR

   2) Mail or drop off your check at the office in your area.
